Register with the surgery

Catchment area

Before filling out a registration form, check you are in our catchment area.

You can also find out which GPs you can register with on the NHS website.

The surgery maintains the right to change the practice boundary as is deemed necessary to maintain the high standard of care offered to existing patients.

Patient removals

All patients who move outside of the stated practice boundary, will be asked to find a new surgery, except those patients who move to Lower Pilsley, Grassmoor, Holmewood and New Tupton only. This is to help us maintain the high standard of medical care which we provide for the immediate locality. We understand that this can cause some distress to some patients but unfortunately we need to be very strict on this so as to ensure that the limited amount of resources available within the practice are utilised within the practice area of North Wingfield.

Registering with the surgery

To register with the surgery:

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Out of area patients

You may be a patient that currently does not live within the practice area, which means you may not be able to access the care that you need. For example:

  • We are not able to visit you at home for your care. If you need a home visit, you will be asked to contact 111 to speak to a medical advisor who will arrange a home visit for you if it is needed.
  • You may not be able to access community care services in your home. 
  • You will not be able to access urgent care from us and if you contact us for this reason, you will be asked to attend a local urgent treatment centre or call 111.
